[PT2021] Interest in CPHS Committees Sign-Up Now Open

Hello Students!

Are you interested in serving as a student representative on a CPHS committee? If so, sign-up for interest is now open! Follow the directions in the form below to show your interest. Forms must be submitted by August 24th. If you would like to look through a list of committees available for student representation, please see the attachment for more information.

Even if you have already served on a committee, we ask that you complete this from again for this year and select the committee. If you do not complete, Student Affairs will assume you are not interested in serving on that committee any longer. Reach out to us with any questions.
