[Pt2024] Clinical Education Action Items due 4/27/22

Good evening,

Please see below for three action items the clinical education team needs to be completed no later than Wednesday, 4/27/22 at 5pm.

  1.  Upload your OSHA certificate of completion to Exaat. This was a requirement for Dr. Sawyer's class, but also needs to be housed in Exaat.
  2.  Attached is a Consent to Disclose Personal Information Form. Please review the form, complete it (including initialing next to each statement on the first page), and upload to Exaat. This form is required to go into clinical settings.
  3.  Update student information in Exaat:
     *   Login to Exaat and navigate to "Edit Student Info". Once there, please confirm that your current, legal name and other information (gender, last 4 digits of SSN, birthday, etc.) is correct and complete. Some student accounts have first name listed as last and last name listed as first, which is incorrect. Additionally, if you have gotten married and your name has changed, the account should reflect whatever your current, legal name is. Please make any necessary changes and save.
