This was a requirement for Dr. Sawyer’s class, but also needs to be housed in Exaat.<o:p></o:p></li><li class="MsoListParagraph" style="margin-left:0in;mso-list:l0 level1 lfo1">Attached is a Consent to Disclose Personal Information Form. Please review the form, complete it (including initialing next to each statement on the first page), and upload to Exaat.
This form is required to go into clinical settings.<o:p></o:p></li><li class="MsoListParagraph" style="margin-left:0in;mso-list:l0 level1 lfo1">Update student information in Exaat:

correct and complete. Some student accounts have first name listed as last and last name listed as first, which is incorrect. Additionally, if you have gotten married and your name has changed, the account should reflect whatever your current, legal name is.
