[PT2021] Remedial Work

Some of you had to do some remedial work. That took the form of a quiz since many problems were decision-based rather than skill-based.

In any case, for the benefit of those remediating and those not, some common errors were apparent that I thought might be best shared with all.


1)      Weakness begins immediately with a stroke. It may worsen after a few days secondary to hemorrhagic transformation or penumbra expansion,  but it definitely does not just appear a few days after the stroke. In many cases improvement in strength may be noted after a few days. So it is important to measure strength when you first see a patient a day or two post stroke. Spasticity, on the other hand, may take a week or so to develop. Subluxation, a complication, may take at least a few days. Consequently, examination of either spasticity or subluxation is likely to be uninformative and thus a waste of time 2 days post stroke.

2)      Low friction fabric is not a good option for moving a limb on and off a bed. It is good for assisting hip abduction and adduction movement on the bed, but it will not help get it back onto the bed.

3)      The best unilateral assistive device for assisting gait is probably the forearm quad crutch. It provides a more stable additional base than a single point cane. Yet, it is easily placed.
