[PT2021] Consent and Release to disclose Personal Information / I 802 Information

Good evening Class of 2021,

Hope everyone is well and enjoying your time before clinicals begin.  I have attached a Consent and Release to Disclose Personal Information Form if you can sign and upload into Exxat under your required documents under Consent and Release Form by Friday if possible.  If you have received your Covid Vaccine I have also opened a tab under your required documents where you can upload proof of vaccine.

At this point you should have all emailed your CI with your introductory email to inquire as to hours, attire, parking and obtain final clearance for onboarding to confirm your start date of April 5. Please let me know if you have not heard back or corresponded with your CI's or in case of MFA sites the managers for each location.

I am so excited for your next stage and wish you luck on your clinicals.  Graduation is almost here!
