[Phar25] Reminder to complete: COVID Vaccination Self-Report and Return to Campus Testing Information

Greetings CPHS students,

The fall semester is right around the corner.  In preparation for the fall semester, the following information is relevant to all students, including all CPHS on-campus programs.

Campbell will require all students to show proof of full vaccination* using the University Self-Report form OR have tested negative for COVID-19 within 48 hours of returning to campus.

*People are considered fully vaccinated: ± 2 weeks after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines, or 2 weeks after a single-dose vaccine, such as Johnson & Johnson's Janssen vaccine.
