Good Afternoon All,

Please find attached the Rotation Hardship form. If you plan to request a rotation hardship, the deadline to submit your completed form and all supporting documentation is Friday, November 3rd at 5pm EST. Please return all documents to me by email or you can drop them off in my office (Room 126 - same hallway as Ms. Debi).
