[PT2021] Answer to Where to upload Consent and Release to Disclose Personal Documents

If you are having any trouble uploading the "Consent and Release to Disclose Personal Documents" the instructions are listed below.

INSTRUCTIONS TO UPLOAD DOCUMENT


1.       Sign the form attached (must include date and signature)

2.       Login to your Exxat Account

3.       Go to Manage Required Documents (maybe under Information Status)

4.       Go to #17 "Consent and Release to Disclose Personal Documents" and upload your signed document.

If you have received your Covid Vaccine you can also upload proof of vaccine (#16).
