[PT2021] Updated form for Flu Vaccine - use this form

Please use this form next week instead of the ones I had previously forwarded as they had to update the form to include the correct lot number of the vaccines they will be using.


1st year students:   Monday, October 21  Room 323 11:30-1pm

These dates work for me! The students will need to review the attached VIS and print and fill out the VAR. I can bring extra copies when I come as well. You don't need to schedule the students, they can just come and go when they are available during the time block.

Wonderful!  I have confirmed the times listed below with Kim and will send these dates out to our DPT 1 and 2 students. If you can have at least 50 shots for each date to cover some for faculty/staff that would be greatly appreciated.  I can stagger the students schedules to come every 10 minutes if you like.  How many students can you handle at each time?  Is there anything else I need to send you or is there paperwork the students need to fill out?

Thank you for your email. I do all of the flu shots, so you don't need to schedule anything with Ann. Unfortunately, I cannot do either of the dates you gave me because I am already booked with clinics on both of those days. What other options do you have available?

I am the new Clinical coordinator for DPT and will help in scheduling the DPT 1 and 2 students to receive their flu shots.



Since this is my first round for setting this up, I may have lots of questions or need help myself :) .  For scheduling, do I need to just book with you or do I need to copy Ann Parker as well?



Are you available on Monday, October 7 from 11:30 am - 1 pm for PT1 Cohort and available on Wednesday, October 16 from 12pm-1:30 pm for PT2 Cohort?

Hi Jennifer,



I've chatted with Katie about this and believe Sarah coordinated this last year.  I have copied Dr. Shearin on this email as I am sure she remembers better than I.  I am happy to work with you on this or do whatever is necessary to help.



She needs about 1.5-2 hours per class, but it doesn't have to be on the same day. If you want to do it in the same day, Katie can probably do both classes in about 2.5-3 hours. To do it that way, Monday or Wednesday afternoons are best for her schedule.
