Please make sure that you return the executed Consent to Disclose Personal Information (FERPA) back to me by tomorrow.  You can drop it off on my desk if I am not in.  My desk is located directly in front of Dr. Dedrick's office.

If you did not receive the Betsy Johnson Parking and Student Parking Decal Request form from me today and you are completing your DPT726 clinical at this site this summer, you can pick up a copy from my desk and deliver to Betsy Johnson security the day of your clinical.
