[PT2021] Walking program supplies

Hi all,

I visited Carillon yesterday and discovered that the supplies are hanging on the back of the director's door.  One bag has the notebook and the other bag has the gait belts.  We have removed 2 residents from the program per their request.  If a resident declines to walk please document the reason why:  tired, in pain, busy at bingo, etc.  Additionally, please be aware that there are 6 lines on the participant forms but only 5 days are needed.  The first line is for the number of weeks.
