If you are interested in participating as a student representative on a CPHS Committee, please complete the survey below to indicate your committee preferences and reasoning.  The committee charges are set forth by the Dean's office.  Filling out this survey does not guarantee you will be the student representative, but we do consider and it helps us to identify candidates. If selected, you will be notified via email by mid-September.
Please note you must be in good academic standing to service on a committee.
