Hello Participant,

We are asking for your assistance in gathering information about current pharmacy student knowledge on factors that impact community pharmacy reimbursement and financial implications. Your participation is completely voluntary but would be highly appreciated.

Your participation would require you to fill out a short questionnaire that will take no longer than 15 minutes to complete. By completing this survey, you are providing your consent to participate in this project. The information that you provide will be kept confidential and will only be accessed by the research team. For presentation and/or publication purposes, all information will be presented in the aggregate and no individual will ever be identified. You are by no means required to finish the survey if you do not wish to and all your information will be confidential and anonymous. If you would like to take this survey, click on the link below. The Midwestern University Institutional Review Board has reviewed this project and determined that it qualifies as an exempt research study.
