[Pa26] sample docs and activity

HI all!

I have posted some sample surgical documents to Blackboard as well as the activity that we ran out of time for.  With the activity, start with the "CASE for surgical documentation activity" document and then start with Group A document, then Group B and so on.  It's great practice to think through these components before the Hospital SIM OSCE on Friday-Tuesday.  I have also posted the documentation rubrics as well.
