Good morning,

While students are signed up for our various communication services at the beginning of each year, I have created a request form where students can sign up to receive access in the event that they have been unsubscribed for any reason or have lost access to any of these services:


  *   The Student Affairs page on Blackboard.
  *   Mass cohort emailing.
  *   The CPHS Weekly Newsletter that is emailed every Wednesday.

Please distribute this form to your students to ensure that all have been given the opportunity to receive access to these services. This form will close on September 30th. Thank you!
