Please be sure to ASK FOR DOCUMENTATION when you get your shot and they will provide you with a card that you can upload into CastleBranch to meet immunization requirements.

Hello Everyone,
On behalf of the Academy of Student Pharmacists (APhA) Operation Immunization, I wanted to send out some information about upcoming flu shot clinics in case you want to receive your flu shot on campus. Please complete the attached form (the Influenza VAR) and read the attached vaccine information sheet in preparation for receiving your shot. Also, please have your insurance card or information handy when coming to the clinic. The location for the flu shot clinics are as follows:



Wednesday, October 8, 1 to 3:30pm, Maddox 117
Monday, October 13, 2 to 5pm, Maddox 130
