Good Morning MS-2 Student Doctors!

If you are receiving this email, you have an OUTSTANDING B6 Final OSCE Long Form Self Evaluation form waiting for completion! If you are confused by which form to complete because you see 2 Long Form evaluations because there were 2 cases just pick one (or if you see a date associated with the evaluations pick the date that is associated with your OSCE date), they are both very similar in nature. Please make sure to have the evaluation completed by Monday 12.8.28 by 8:00 am so we can process grades! If you have any questions, please let me know.

In order to receive a final grade for the course, students must complete all of the following components of the Final Summative OSCE:

1.   Final Summative OSCE experience;
2.   Short Form Self-Evaluation; and
3.   Long Form Self-Evaluation.

If a student does not complete all required components as stated above by the designated due date, they will receive a grade of INCOMPLETE for the course until all required assignments are submitted.  This may also prompt a referral to the APPS Committee and may prohibit the student from continuing in the remaining portion of the academic year.
