The below email is sent on behalf of the Surgery Club.

Hey all,

Below are the instructions for registering for Surgery Club, please do not hesitate to contact me if you have any questions! I have also attached a document to outline the differences between the National and Local dues options. Quick summary is that the Local gets you access to all CUSOM driven surgery club events and National gets you local + mentorship, conference access, and specialized events run by the national ACOS-MSS committee. Once you have paid your dues please use the google form to submit a screenshot of payment, this is the best way for us to make sure you are added to the roster promptly!

Instructions:

Two options:

 1) Local dues ($45 total): $45 using this link<https://igfn.us/form/VkDPug> (select the option that says "local dues only")
Please complete the attached google form<https://forms.gle/NgQvB544FmCQ8XJF8> indicating which sub-interest groups you are interested in AND upload a screenshot of the receipt page after paying the local dues ($45).

OR

2) National dues ($60 total):
2a) $30 using this link<https://igfn.us/form/VkDPug> (select the option that says "local and national dues- $30)
                          AND
2b) Register with the ACOS-MSS National <https://www.facos.org/OS/Navigation/Membership/Student_Join/Student_Join.aspx> ($30)
2c) Please complete the google form<https://forms.gle/NgQvB544FmCQ8XJF8> indicating which sub-interest groups you are interested in joining AND upload a screenshot of the receipt page after paying the national dues ($30)
