Timely compliance and <i>documentation of compliance</i> with immunizations are essential for the health and safety of you and your patients.<o:p></o:p></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;text-align:justify;line-height:115%">
On the first day of each month, Clinical Affairs confirms student compliance with immunizations through the CastleBranch platform. Students who are not compliant with<i> receiving and submitting documentation for</i> required immunizations are placed on
<b>conduct probation at CUSOM.</b> Please remember that you must receive the needed immunizations AND submit documentation to CastleBranch that you have been vaccinated. As in clinical medicine
<i><u><span style="font-size:14.0pt;line-height:115%">“if it is not documented, it did NOT happen.”</span></u></i><o:p></o:p></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;text-align:justify;line-height:115%">
Students who are non-compliant with immunizations are placed <b>conduct probation</b> and have 14 business days to receive the needed immunizations.
<b>Students who remain non-compliant after 14 days are referred to the APPS Committee.<o:p></o:p></b></p>
