Good Morning and Happy Friday!

Please be aware that if you experience any kind of needle stick, fluid exposure, etc. you are to complete the attached form and return it to me within 72 hours. This form can also be found under "Forms" in New Innovations.

More information about this policy can be found beginning on pages 159-161, of the Academic Bulletin.

Once the form is submitted to me, I will forward it on to the Campbell Health Center.
