[CUSOM Immunizations]

Good morning,

I hope this email finds you well. As a reminder, you are required to keep all immunizations and annual renewals current during the entirety of medical school. Please make sure your CastleBranch account is current, including, but not limited to the following:

1.      Health Insurance Renewal - Due annually on September 1st
2.      TB Renewal - Due annually
3.      Flu Renewal - Due annually, by October 15th

As stated above, your annual health insurance renewal is due by September 1st. Please make sure your health insurance and any other overdue items are completed and accepted by CastleBranch by September 1, 2024. It is important that you confirm your upload was accepted by CastleBranch. If it is rejected, for any reason, it is your responsibility to correct the problem.

Failure to do so will result in referral to the APPS Committee.
