Hello all,
The Campbell University Community Care Clinic is planning on holding a fundraiser to help fund the work we do as a free clinic for underserved members of our community. We are interested in selling Patagonia jackets with the school logo embroidered on the front. In order to initiate this fundraiser, we need an estimate as to how many people would be interested in participating. Please complete the following short Wufoo form if you are interested:

This is not an official order form, we will be sending the official order form out early to mid October. Pricing will be based on generated interest but is estimated to be around $100-$150. The jacket style will be based on the most popular selection from the Wufoo form and will come in the grey color. Collection of data from this form will conclude on September 27th.
