[Cusom26] Medical Student Jacket Interest Form

Hello CUSOM Students,

My name is Ashlyn Acheson, and I am the current SGA Secretary!

I am working with the Campbell bookstore to get us a Patagonia/Jacket that does not have a club name on it. Before they are able to get the final pricing of the product, they need an estimate of those that are interested in purchasing. Our price range is $100-200, and it will be a full zip jacket with the CUSOM logo. There will also be an option to add your name to the jacket if desired.

Here is a google form. It is an interest form. Please submit by Monday morning so we can get this order process rolling 🙂
