[Cusom26] Affiliation Agreement Reminders

As you are applying for away rotations, please remember to submit your affiliation agreement request via the Wufoo form.  We only need one request per program, even if you are applying for multiple rotation dates within that program.  Additionally, please remember to update me with any confirmed rotation start dates as soon as possible.
