[Cusom26] CastleBranch Immunization & Documentation Deadline: October 7, 2022

Good Morning All,

I hope you are adjusting well to your classes and enjoying your time at CUSOM. Please note: all immunizations and required documents should be uploaded to your CastleBranch account by October 7th. The only exception is if you are currently completing a Hepatitis B series; due to the time required between doses. If you are completing your Hepatitis B series, please update me on your progress and make sure to follow your deadlines as closely as possible.

As stated in the CUSOM Academic Bulletin, students must obtain and remain compliant with all immunization requirements through graduation.

This email is to notify you that the deadline for completing this requirement for the 2022-2023 academic year is October 7, 2022. Please submit all required documentation in CastleBranch no later than October 7, 2022. Failure to meet this deadline will result in a referral to the APPS committee.

It is important to confirm all required documents were upload correctly and accepted by CastleBranch. If there are any issues or errors with your documentation, CastleBranch will provide details on how to correct the issues in your account. You must return to your account and confirm all documentation uploaded successfully.
