[PT2021] COVID vaccination proof in Exxat

Hello,

A duplicate column was made for the COVID-19 vaccination when I updated requirements for monitoring immunizations in EXXAT.  This happened just in the past few weeks.  We had to eliminate the duplicate and several students had placed immunization records in the new column which has been deleted.  Please access EXXAT by September 15th and ensure that your record is still uploaded, or re-submit.  My apologies for the error.
