<p class="MsoNormal">I am sending out the information for you about the flu shot clinic for pharmacy students coming up next week. The clinic will be held on
<b>October 13<sup>th</sup> from 1-5pm in 228 Maddox. <o:p></o:p></b></p>

<p class="MsoNormal">Please note, we are reserving the 1-2pm hour for people who have electives beginning at 2pm. P1 students should come
<i>after</i> PHRD 521.<o:p></o:p></p>
<p class="MsoNormal"><o:p> </o:p></p>
<p class="MsoNormal">Please see the attached VAR and VIS forms. If you have a primary care provider, fill out this section completely
<i>(including fax number)</i> as we are required by the BOP to notify them within 72 hours that you received your flu shot. If you do not have a PCP, check off the box provided. If you use the CU Health Center, you can simply write that under “Name of Medical
Practice” and leave the rest blank.<o:p></o:p></p>

<p class="MsoNormal">If you are unable to attend the clinic during this time, you may get your flu shot at the pharmacy of your choice and upload the documentation they provide you into Complio. They are due into Complio by Oct 29.<o:p></o:p></p>
